Does MP4 to MOV lose quality?

Very little. The video is re-encoded with the codec, resolution and bitrate you choose; keeping the source codec and bitrate (the default) produces a MOV that is visually the same as the original. Lowering the bitrate or resolution is what actually reduces quality.
